Elder Care Coordination

Clients can purchase Elder Care Coordination for themselves or for a family member.  We offer three Elder Care Coordination packages, 6 month, one-year and lifetime. An Elder Care coordination package includes all services offered by EAG for the period of time selected. Clients who purchase on of Elder Advocacy Groups ala cart packages have the option of applying those fees to an Elder Care Coordination package.

All Elder Care Coordination packages include:

  • A written Elder Care Coordination plan with binder
  • Regular visits to client and meetings with family as preferred
  • Home Care Planning
  • Crisis Intervention
  • Placement Assistance by touring, evaluating and choosing service options
  • Care Transition Assistance
  • Bedside Patient Advocacy
  • Review of plan (annually for a lifetime package)

Assistance With Transition To Home – One discharge ($289)

The discharge process can be one of the most stressful parts of returning home after an illness. Elderly patients returning home from a hospital stay often have multiple orders, treatments and healthcare professionals to coordinate. Handling all of these moving parts can be difficult for anyone to handle. Our Elder Care Coordinator will visit our client at the facility and liaison with the discharge planner to ensure all everything is planned for.  We will communicate with the client and their support system to make sure everyone has their questions answered. On the day of discharge, we will follow the client home to ensure a smooth transition. In seven days we will follow-up with a visit or phone call to make everything is going as planned.

If an elderly client is changing home environments, our Elder care coordinators can also help with transition to an assisted living or nursing home.

Any Transition Assistance service fee can be applied to any size Elder Care Coordination Package.

Placement Assistance – 3 tours ($395) 5 tours ($595)

Because Elder Advocacy Group doesn’t take referral fees from facilities or home care companies, you can be certain we will show our clients only the best services available.

Add Transition Assistance to this plan to guarantee a smooth transition into their new home environment. Any Placement Assistance fee can be applies to an Elder Care Coordination Package.

Crisis Intervention – $750

Sometimes aging can catch up with an elder very quickly. What seems like a little trouble keeping up with chores around the house can develop into a potentially dangerous situation. If this has happened to someone you care for then Crisis Intervention might help. Our Elder Care Coordinator can do a home assessment to determine whether an elder is in safe living environment. After that our Elder Care Coordinator will work with the elder and their family to put services in place to help them stay at home. If that is not possible we will help the elder find a new safe place to call home.
